For psoriasis, only individual small studies exist. A controlled trial with cannabidiol oil showed no improvement in skin findings, though it did show relief of itching. Overall, the evidence is preliminary and insufficient for an assessment of benefit.

Rating scheme

The letter rates the quality of a study, independently of its type. Every study type can receive any grade: a review can be B or C when it is small or weak, and an RCT can be S. The grade is a synthesis of study design, journal authority and clinical bindingness:

Systematic review on cannabinoids in autoimmune and inflammatory skin diseases; k=15 clinical studies (SSc, DM, Psoriasis, AD) from 389 screened articles. Evidence suggests potential benefits for psoriasis and other indications, however further RCTs are needed. Mechanisms via the endocannabinoid system and anti-inflammatory properties are discussed.

Efficacy and safety of cannabidiol oil in psoriasis: a randomized, double-blind, placebo-controlled trial.

Key finding

CBD oil did not lead to a significant improvement in psoriasis severity (PASI scores), but showed temporary improvements in itch and sleep onset latency.

Summary

n=28 chronic plaque psoriasis, oral CBD oil 60 mg/day vs. placebo over 12 weeks. Primary outcome PASI score: no significant improvement. Secondary outcomes: significant itch reduction up to week 8, shortened sleep latency up to week 6 (not sustained). Adverse effects mild to moderate, comparable between groups.
