Individual small studies on topical or oral cannabidiol report a reduction in itching, mostly as a secondary outcome. Larger trials are needed for a reliable assessment.

Itch intensity (NRS-11) decreased significantly by 55%, in 75% of participants ≥2-point improvement. Significant improvement in physiological skin parameters, no adverse events. Primarily atopic dermatitis — pruritus evaluated as core symptom.

Efficacy and safety of cannabidiol oil in psoriasis: a randomized, double-blind, placebo-controlled trial.

CBD oil did not lead to a significant improvement in psoriasis severity (PASI scores), but showed temporary improvements in itch and sleep onset latency.

Summary

RCT (n=28) on oral CBD oil 60 mg/day vs. placebo in chronic plaque psoriasis over 8 weeks. Primary endpoint (PASI) not significantly improved. Secondary: significant itch reduction (itch score) up to week 8; sleep onset latency decreased up to week 6 (not sustained). Adverse effects mild to moderate, comparable to placebo. Primarily psoriasis — pruritus as secondary outcome.

An Observational Study of the Application of a Topical Cannabinoid Gel on Sensitive Dry Skin.

n=16 patients with self-reported atopic dermatitis, topical CBD gel over 2 weeks; POEM score decreased significantly from 16±1.35 to 8.25±1.80 (p<0.0007), QOLHEQ emotional domain from 20.9±2.06 to 8.375±1.609 (p<0.004). 67% reported itching reduction, 50% >60% improvement of eczema. No control group, primarily atopic dermatitis — pruritus as secondary symptom.

The patient showed an improvement in pruritus score from 7/10 to 3/10 under oral cannabinoids with initially increased sedation, which however developed quickly.

Summary

Single case report (n=1) on refractory neuropathic pruritus in an ALS patient; oral cannabinoid (balanced THC/CBD) reduced pruritus score from 7/10 to 3/10. Initial sedation, rapid development of tolerance. No systematic evidence, pure case series.
